Always ask before you list someone. It's polite, and it lets them get ready for a call.
Ask clearly. 'Would you be a reference for me when I apply for jobs?'
Give details. Tell them what kind of job you're seeking.
Get their info. Ask for the phone number or email they want employers to use.
Say thanks. Thank them for helping you.
